I think that is because we have left it off the table of modern Christianity. Though he is advocating 'flat' leadership, he also points out that, at certain times, those with the gifts of leadership or prophecy or whatever must step forward and assert 'control' as they utilize their gifts, always for the growth of the community of faith. It is hard, however, to develop the level of trust that is necessary to have no hierarchy but that allows people to step forward at the right moment!
